Step 1: Check you need to make a referral
Before referring a resident to Adult Social Care, make sure you do not need help from other services:
- housing - council or sheltered housing from district or borough councils
- transport - community transport options
- equipment and adaptations - search our online equipment catalogue for ideas
- social - if someone is feeling lonely or isolated
- mental wellbeing - to help with things like bereavement or depression
- local community support - to find local support
If these options are not relevant, continue to Step 2.
Step 2: Make a referral to Adult Social Care
Only a small percentage of our professional referrals result in residents getting support from us. In many cases, this is because individuals didn’t meet the criteria. This can lead to confusion and frustration for local people, their families and carers. Before making a referral, please check the relevant eligibility criteria.
Make sure to send our Paying for care information to the person you're referring. This will help them to understand that they may need to pay towards their care.
Adult with care and support needs referral
Before you make a referral, make sure the person you are referring has eligible adults care and support needs.
Carer with support needs referral
Before you make a referral, make sure the person you are referring has eligible carer's care and support needs.
Mental health referral
Use this form to refer someone into the county council's mental health social care team. If there are concerns for someone's mental state or they need crisis support, please refer to the appropriate .
Safeguarding referral
Use this form to refer someone with care and support needs who is at risk of abuse or neglect.
Report adult social care provider quality concerns
Use this form to report care quality issues that may not meet the safeguarding thresholds.
Deprivation of Liberty (DoLS)
Request standard authorisation or urgent authorisation.
Advocacy
You can refer people directly for advocacy services.
